Varicocele
One of the possible reasons for a low sperm count is a varicocele, which is a swollen varicose vein in the scrotum - usually on the left side. The reason for infertility associated with a varicocele is unclear. It may also have a progressively damaging effect on sperm production, so that the sperm count may decline with time.

Most men are not keen on semen testing in the first place, and a diagnosis of low sperm count puts extra pressure on him. Since there is no guaranteed treatment to remedy this condition, it is preferable to try alternative solutions like such as IVF and ICSI.
