Patient: Dr. Malpani, I’ve read that embryo transfer is one of the most important steps in IVF. Is that true?

Dr. Malpani: Yes, absolutely! Embryo transfer is the final and most critical step in your IVF journey. While we help you grow eggs and create embryos in the lab, pregnancy can only happen once we transfer those embryos into your uterus. It’s an exciting and emotional milestone.

Patient: That makes sense. I’ve heard some people say they get to see their embryos before the transfer. Is that possible?

Dr. Malpani: Yes, and I strongly recommend it! Seeing a photograph of your embryos can be very special—it helps you connect with them. Many patients even call them “embies” because they represent the potential life growing inside you. More importantly, embryo photos give you an idea of their quality and the chances of implantation.

Patient: That’s fascinating! But how do I know if my embryos are good quality?

Dr. Malpani: Your doctor should provide detailed information about your embryos’ grading and appearance. There are specific criteria we use to assess embryo quality, such as their shape, the number of cells, and how they are dividing. If your doctor doesn’t automatically show you the pictures, I encourage you to ask for them. It’s your right as a patient to know exactly what is being transferred.

Patient: Thank you! Now, can you walk me through what actually happens during the embryo transfer?

Dr. Malpani: Of course! The procedure is quite simple and painless. First, you’ll need to have a full bladder. This helps us get a clear ultrasound image of your uterus, which allows us to guide the embryo transfer catheter accurately.

Patient: A full bladder? That sounds a little uncomfortable.

Dr. Malpani: It might be slightly uncomfortable, but it’s important for us to visualise your uterus properly. The good news is that the procedure is quick—it only takes a few minutes. Once your bladder is full, we gently insert a thin, soft catheter through the cervix into the uterus under ultrasound guidance.

Patient: Does the ultrasound help you see the embryos being placed?

Dr. Malpani: That’s a great question! No, the embryos themselves are microscopic, so they don’t show up on the ultrasound. However, we mix them with tiny air bubbles in the transfer medium. During the transfer, you’ll see a little flash on the screen—that’s the air bubbles moving into your uterus.

Patient: Wow, that’s amazing! And where do the embryos go once inside?

Dr. Malpani: The goal is to place them in the optimal position within the uterine cavity so they can attach to the lining. Once transferred, the embryos settle naturally, and we hope for implantation in the following days.

Patient: That sounds so precise. What happens immediately after the transfer?

Dr. Malpani: After the transfer, you’ll rest for a short while, but there’s no need for extended bed rest. You can go home and continue your normal activities, avoiding strenuous exercise or heavy lifting. The waiting period after the transfer—known as the two-week wait—is the most challenging part for many patients.

Patient: Yes, I’ve heard about the two-week wait. Is there anything I can do to improve implantation chances?

Dr. Malpani: The most important thing is to stay relaxed and take good care of yourself. There’s no special diet or activity that will guarantee implantation, but maintaining a positive mindset, eating a balanced diet, and following your doctor's instructions can be helpful.

Patient: That makes sense. When will I know if the embryo has implanted?

Dr. Malpani: About 10–14 days after the transfer, we’ll do a blood test to measure your beta-hCG levels. This will tell us whether the embryo has implanted successfully.

Patient: I’m feeling a mix of excitement and nervousness! What if it doesn’t work?

Dr. Malpani: That’s completely natural. IVF is an emotional journey, and while we do everything possible to maximise success, implantation is ultimately beyond our control. But remember, each cycle teaches us something, and if needed, we can adjust our approach for the next attempt. Stay hopeful!
